The side includes deadly wing Josh Drauniniu who scored a stunning hat-trick last week and signed a new long-term deal at Warriors this week.
Warriors make two changes on the bench as Scotland international Euan Murray, who was named in his country's training squad this week, is named among the replacements. He is joined by experienced lock Chris Jones.
Worcester claimed an 85-3 victory over Bizkaia Gernika RT when the teams met back in October, but Assistant Head Coach Nigel Redman vowed his team would give the Spanish side respect.
"They scored two tries last weekend against Perpignan and only let in 50 points which I think is very respectable in as far as the side the French put out," he said.
"The Basque Country, whether you are in Northern Spain or Southern France, is a very proud rugby region.
"They have beaten Rovigo home and away so they have got an awful lot out of this tournament. They will come to Worcester keen to give a very good account of themselves and it is important for both teams that they do that.
"The big focus for this week is to improve our accuracy, particularly the pass, running line and understanding the roles they adopt in our attacking game," he added.
"There are objectives for this week so we are looking to really step it up. The team played together against Rovigo and now we need to move up a level against Gernika."
